How we calculate this score

Every regulation HIQA judged on an inspection earns points: Compliant 100, Substantially compliant 65, Not compliant 25.
Where the compliance plan rates the risk of a failing regulation, orange (moderate risk) takes off 15 more points and red (high risk) takes off 40. Yellow (low risk) takes off nothing.
An inspection's score is the average over the regulations it looked at.
A home's score combines its last 3 inspections: the newest counts for 50%, the one before for 30% and the one before that for 20%. With fewer inspections the weights are scaled up to 100%.
The trend compares the newest inspection's score with the average of the earlier ones: 5 points or more higher is improving, 5 or more lower is declining, anything else is stable.
Homes are ranked by score within their county and among scored homes within 25 km. Homes with the same score share a rank.

Method v1. It uses only HIQA's published judgments, not our opinion, and is recalculated whenever a new report is published.

What the latest inspection found

Beech Lodge Care Facility is a 66-bed nursing home in Bruree, Limerick, offering long-term, respite, convalescent, palliative, and dementia care. Residents generally feel comfortable and well cared for, praising the kind and attentive staff and the social dining experience. However, inspectors found issues with staff supervision, especially for residents needing one-to-one care, and inconsistencies in record-keeping and care planning. Activities are provided but not always tailored or consistent, particularly in the dementia unit. Nutritional care does not always meet prescribed dietary needs.

Long-term care

Palliative care

What inspectors liked

Residents expressed high satisfaction with the supportive and familiar staff team.
Nursing staff were described as kind, attentive, and responsive to call bells.
The dining experience was social and positive, with good food quality and choice.
There is 24-hour nursing care with trained staff and access to GP and physiotherapy services.
Complaints were managed effectively and resolved appropriately.

What needed to improve

Residents requiring one-to-one or enhanced supervision were sometimes left unsupervised as staff were assigned other duties.
Care plans were not always updated to reflect changes in residents' needs, especially nutritional and wound care.
Records of medical assessments, treatments, and nursing notes were incomplete or duplicated, affecting care quality.
Therapeutic diets prescribed by health professionals were not consistently provided to residents.
Activities were not consistently available or tailored, especially for residents in the dementia unit or those needing individual engagement.

What residents said

“Residents described feeling comfortable and happy with the direct provision of care.”
“Residents spoke positively about nursing staff, describing them as kind and attentive.”
“Residents reported their bedrooms were comfortable with adequate storage.”
“Residents felt safe living in the centre.”

Staffing: There is 24-hour nursing care with a team of highly trained staff. However, inspectors found that supervision was insufficient, with staff assigned to one-to-one care often performing other duties, leaving residents unsupervised at times.
Activities: An individual programme of activities is tailored to each resident, including newspapers, exercises, brain games, music, mass, art, baking, hairdresser, bingo, and sensory therapy. However, activity provision was inconsistent, especially in the dementia unit and for residents needing one-to-one engagement.
Food: Meals are wholesome, nutritious, and served attractively with choice offered. Catering staff are aware of residents' preferences and supported by dietitians and speech and language therapists. However, therapeutic diets prescribed were not always provided as required.
Dementia: Has a dedicated dementia unit. The centre provides care for residents with dementia. However, residents in the dementia (Daffodil) unit did not have consistent access to appropriate and meaningful activities, with extended periods of limited engagement observed.
